Who is Eligible for Free Care at Grace Clinic?
Grace Clinic is committed to serving residents of Benton and Franklin Counties and Burbank, who have no medical insurance and income at or below 200% of the federal poverty level.
All services provided by the clinic are free of charge to patients. We do encourage patients to give back through a small donation.

Patient Eligibility Requirements
To establish care at Grace Clinic you must follow these general requirements:
- Be uninsured and not eligible for a government sponsored program
- Income must be at or below 200% of Federal Poverty Guidelines
A potential patient must come to Grace Clinic during clinic hours and be screened for eligibility. At this time, all dental appointments are made after being screened for eligibility on Thursday's between 7:30-10:30am (except the second Thursday of the month).
What to bring:
- Proof of Income for the past 30 days
- Photo identification
- List of current medications
- Copy of all past medical records is desired. (if you have seen a physician in the last three years)
